Every preventive care visit you complete this year gets you entered into a drawing. There are three drawings — end of July, end of September, and mid- December — so there’s always a reason to submit an entry.
What counts:
- Annual physical or wellness exam
- Preventive screenings (mammogram, colonoscopy, and others)
- Dental exam or cleaning
- Preventive bloodwork or a lab panel
To enter, just let [Nasim / HR contact] know the type of visit and the date. No medical details, no records — just the visit type and date, on the honor system.
